After coming through the youth ranks at River Plate, who he joined as a youth player in 2016, Alvarez made his professional debut with the Argentine club's first team on 27th October 2018. A few weeks later he won the Copa Libertadores, coming off the bench during extra time in the final. He scored his first competitive goal on 17th March 2019 and, over the next few years, went on to win the Copa Argentina, Supercopa Argentina, Primera División and Trofeo de Campeones. On an individual level, Alvarez was the top scorer in the Argentinian league in the 2020/21 season with 18 goals. He finished his time at River Plate with 54 goals and 31 assists in 122 games.

In 2022 he joined Manchester City, where he has won two Premier League titles, the Champions League, FA Cup, European Super Cup and the Club World Cup, in which he was the top scorer with two goals. Across two seasons in England, he played 103 games and recorded 36 goals and 18 assists.

He made his official debut for Argentina's senior national team on 3rd June 2021, lifting the Copa América trophy the following month, before winning both the CONMEBOL–UEFA Cup of Champions and the Qatar World Cup in 2022. This summer he won the second Copa America of his career and also represented his country at the Olympic Games. To date, he has earned 36 caps for Argentina, scoring nine goals and providing one assist.

The striker completed his medical examinations at the Centro de Medicina Deportiva de Alto Rendimiento Vithas - Invictum at the Hospital Universitario Madrid Arturo Soria.
